Direct Debit Set Up
You need to fill and sign in the form below. Click download link
Take a photo of completed form and email it back to us at the email admin@uptonmusicschool.co.nz
When we receive the completed form we will then send it off to get authorised by your bank.
Once they confirm authorisation we will set up the weekly amount you want to debit. This will come out automatically each week/month/term.

Please note direct debits incur 6% admin fee. Scenarios shown include admin fee.
Group fee $190 term fee.
- Weekly fee over 8 week term and 2 week holidays $20
- Weekly fee with 1 year commitment (52 weeks) locked in $15
- Weekly fee over the 8 week term – $25
